SUMMARY: The dentogingival junction (DGJ) is an adaptation of the oral mucosa composed of epithelial and connective tissues intimately related with the mineralised tissues of the tooth. The histological evidence available is mainly based on studies in animals, separate evaluations of hard and soft tissues, and studies using conventional histological techniques that eliminate the enamel from preparations. The aim of this study was to carry out a review of the existing evidence on histological techniques available for study of the tooth and periodontium in conjunction in humans. A scoping review was carried out of the available literature referring to study of the tooth and the periodontium in conjunction in humans, in the Web of Science (WoS), EMBASE, Scopus and SciELO databases, using the terms "Histological Techniques"[Mesh]) and "Epithelial Attachment"[Mesh]. One hundred and fifty-nine articles were found, of which 54 were selected for full- text reading. Ten were finally included in the qualitative synthesis, and we applied the Anatomical Quality Assurance (AQUA) checklist for analysis the methodological quality of the selected articles. The results showed that the only articles with a low risk of bias in all five domains according to the AQUA criteria corresponded to Silva et al. (2011) and Agustín-Panadero et al. (2020). Finally, we conclude that the quality of the histological sections to observe tissues that simultaneously contain the tooth and the periodontium, is conditioned by the selected technique and by the care required in certain specific tasks during the histological processing of the samples.


La unión dentogingival (DGJ) es una adaptación de la mucosa oral compuesta por tejidos epitelial y conectivo íntimamente relacionados con los tejidos mineralizados del diente. La evidencia histológica disponible se basa principalmente en estudios en animales, evaluaciones separadas de tejidos duros y blandos y estudios utilizando técnicas histológicas convencionales que eliminan el esmalte de las preparaciones. El objetivo de este estudio fue realizar una revisión de la evidencia existente sobre las técnicas histológicas disponibles para el estudio del diente y el periodonto en conjunto en humanos. Se realizó un scoping review de la literatura disponible referente al estudio del diente y el periodonto en conjunto en humanos, en las bases de datos Web of Science (WoS), EMBASE, Scopus y SciELO, utilizando los términos "Histological Techniques"[Mesh]) y "Epithelial Attachment"[Mesh]. Se encontraron 159 artículos, de los cuales 54 fueron seleccionados para lectura de texto completo. Diez fueron finalmente incluidos en la síntesis cualitativa, y se aplicó la lista de verificación Anatómica Quality Assurance (AQUA) para el análisis de la calidad metodológica de los artículos seleccionados. Los resultados mostraron que los únicos artículos con bajo riesgo de sesgo en los cinco dominios según los criterios AQUA correspondían a Silva et al. (2011) y Agustín-Panadero et al. (2020). Finalmente, concluimos que la calidad de los cortes histológicos para observar los tejidos que contienen simultáneamente el diente y el periodonto, está condicionada por la técnica seleccionada y por el cuidado requerido en ciertas tareas específicas durante el procesamiento histológico de las muestras.

BACKGROUND: Autogenous soft tissue grafting is indicated in thin gingival biotypes before orthodontic proclination or labial movements to increase the keratinized gingiva and prevent gingival recession. However, its effect on local alveolar bone remodeling is unclear. The aim of this study was to investigate the effects of autogenous soft tissue grafting on local alveolar bone after orthodontic proclination or labial movements. METHODS: Sixteen patients with a thin scalloped gingival biotype, narrow keratinized gingiva, or thin cortical bone requiring orthodontic proclination or labial movement of teeth were included. Cone-beam computed tomography (CBCT) images were obtained before grafting and at least 6 months after surgery. Sixty mandibular teeth were included, and the vertical bone level and horizontal labial bone thickness were measured. The results were compared using paired t-tests or Wilcoxon signed-rank test. RESULTS: The horizontal labial bone thickness increased, especially at 6 mm below the cementoenamel junction (CEJ) in the mandibular central and lateral incisors (P < 0.05). The total alveolar bone area of the canines, first premolars, and second premolars increased at 3, 6, and 9 mm below the CEJ, respectively, and the differences were statistically significant (P < 0.05). Additionally, vertical bone height increased minimally on the labial side, but the differences were not statistically significant (P > 0.05). CONCLUSIONS: New bone regeneration was observed on the labial (pressure) side after autogenous soft tissue grafting, which may represent a mechanism to effectively prevent gingival recession and maintain periodontal health. This study evaluated the efficacy of botulinum toxin A (BTX A) in improving the aesthetic appearance of lips. Twenty-four outpatients with clinical evidence indicating decreased fullness of lips or gummy smile were selected and received BTX A injection on the orbicularis oris. We observed a significant decrease in wrinkles and improvement in gummy smile in all patients 4 weeks after the injection. Aesthetic lines also changed significantly. Local injection of BTX A on the orbicularis oris could simultaneously achieve mild lip enhancement, improvement in fine wrinkles around lips, and mild gummy smile.

La odontología estética no es una disciplina especial o área de la odontología en sí misma, pero con consideraciones funcionales y biológicas, representa uno de los objetivos de las intervenciones de tratamiento dental, que abarca todas las áreas de especialidad, desde la odontología preventiva y restaurativa hasta la prostodoncia, ortodoncia, periodoncia, como, así como la cirugía oral y maxilofacial. El cénit gingival es un importante componente de la sonrisa y su estudio sigue siendo muy relevante en la odontoestética internacional. En el presente trabajo se determinaron distancias del cenit al eje longitudinal y se correlacionaron con otros parámetros gingivales, se establecieron diferencias en las alturas de las papilas interdentales y se correlacionaron los datos métricos de las piezas dentarias anteriores de la población estudiada y los datos conocidos con el fin de obtener datos estadísticos relevantes. Los datos mesurables fueron obtenidos de pacientes de ambos sexos, (18 ­ 25) años, con piezas dentarias del grupo anterior y superior a saber: incisivos centrales, incisivos laterales y caninos superiores, normalmente implantados, libres de lesiones o restaruración, ausencia de enfermedad gingivoperiodontal y sin tratamientos ortodoncicos. El trabajo observacional, descriptivo y tranversal arrojó resultados basados en la estadística preponderante. Provee dimensiones y proporciones de dientes maxilares que pueden adaptarse a pacientes individuales en relación con parámetros establecidos en la odontoestética. Estos datos pueden ser pautas útiles para el diagnóstico y la planificación del tratamiento (especialmente cirugía periodontal) en la dentición maxilar (AU)


A esthetic dentistry is not a special discipline or area of dentistry itself, but with functional and biological considerations, it represents one of the objectives of dental treatment interventions, which covers all areas of specialty, from preventive and restorative dentistry up to prosthodontics, orthodontics, periodontics, as well as oral and maxillofacial surgery. The gingival cenith is an important component of the smile and its study remains very relevant in international dentistry. In this work, distances from the cenith to the longitudinal axis were determined and correlated with other gingival parameters, differences in the heights of the interdental papillae were established and the metric data of the anterior teeth of the studied population were correlated and the known data with in order to obtain relevant statistical data. The measurable data were obtained from patients of both sexes, (18 - 25) years, with teeth of the anterior and superior group, namely: central incisors, lateral incisors and upper canines, normally implanted, free of lesions or restoration, absence of disease gingivoperiodontal and without orthodontic treatments. The observational, descriptive and transverse work produced results based on the preponderant statistics. It provides dimensions and proportions of maxillary teeth that can be adapted to individual patients in relation to parameters established in odontoesthetics. These data can be useful guidelines for diagnosis and treatment planning (especially periodontal surgery) in the maxillary dentition (AU)

The black triangle resulting from interdental papilla (IDP) loss is associated with poor aesthetics and difficulty in pronunciation and food impaction. There is limited knowledge of gingival tissue inflammatory response to hyaluronic acid (HA) filler injection, a minimally invasive IDP reconstruction method. This study aimed to examine the morphological and histological changes in IDP and the inflammatory cytokine localization to the IDP post-HA filler injection using an open gingival embrasure (OGE) mouse model. Mice from the control, sham, and OGE groups were attached with reference, inactive, and activated wires for 5 days, respectively. The degree of IDP loss was determined based on the spring-papilla distance (SPD). Morphological and histological changes in the OGE group injected with phosphate-buffered saline (PBS) or HA fillers were examined on days 2 and 7 post-injection. Immunohistochemical analysis was performed to determine the localization patterns of tumor necrosis factor (TNF)-α, interleukin (IL)-1ß, IL-6, myeloperoxidase (MPO), and Ki67. Five days post-wire attachment, the control and OGE groups exhibited a significantly higher SPD than the sham group (p < 0.0167). The SPD of the HA filler injection group was significantly lower than that of the PBS injection group on days 2, 4, and 7 post-injection (p < 0.05). The IDP of the OGE group was wide and flat. HA filler was stable in the connective tissue underlying the epithelial tissue even on day 7 post-injection. TNF-α, IL-1ß, IL-6, MPO, and Ki67 were highly localized to the connective tissue surrounding the filler on day 2, which decreased on day 7 post-injection. Thus, HA filler can safely and successfully reconstruct the IDP in cases of OGE.

In classical textbooks of Anatomy, the mental nerve is considered to be the terminal or main branch of the inferior alveolar nerve, especially trifurcate with no designated names as soon as it emerges from the mental foramen. The textbooks define the innervation area of the mental nerve regionally without naming its terminal branches. Nomina Anatomica designates 3 terminal branches of mental nerve as "labial, gingival, and mental branch" but offers no description about their distribution on the mandible. In the present study, bilateral dissections were performed on the lower lip specimens of 20 newborns for 40 mental nerves to determine the branching types of mental nerve. Although anatomy textbooks indicate that mental nerve divides into 3 branches, the authors noted that mental nerve branched into 1, 2, or 3 branches at or right after its exit from the mental foramen. Branching patterns were typed in 13 different subclasses under 3 main groups (Form I-II-III). Moreover, the branching patterns that could not be included in one of these 3 main groups were defined as "Complex Form." The most common type of branching that the authors observed was Form II, which had 2 terminal branches with an incidence of 41.9% (16 specimens). The next frequently encountered type was Form III, having 3 terminal branches, and it was detected in 32.0% of patients (12 specimens). The least common form was Form I, which had 1 terminal branch with a frequency of 15.8% (6 specimens). The unclassified group, Complex Form, was found 10.5% (4 specimens).

BACKGROUND: Careful consideration and assessment of the type of phenotype has gained a fundamental importance in the treatment planning for any patient. We evaluated the prevalence of gingival phenotypes in a sample of Yemeni population and to explore its relationships to gender, age and other risk factors. METHODS: This cross-sectional study was performed among 456 patients. All maxillary anterior teeth were included for all parameters and 1st molars were included for gingival thickness measurements. All patients included in this study were systemically healthy and presented no dental crowding. Four clinical parameters were systematically recorded: Gingival thickness (GT), Width of keratinized gingiva (WKG), Crown width/ crown length (CW/CL) ratio and Papilla height (PH). Scores obtained from different parameters measurements were recorded and analyzed using non-parametric tests where P-value < 0.05 was considered significant. One examiner performed all measurements. RESULTS: The mean age was 29.9 ± 8.26 years. Of 456 recruited subjects, 83 (18.2%) subjects had thin, 69 (15.1%) had thick GT and 304 (66.7%) were placed in non-categorized (1.5-2 mm) GT. Square crown shape was found in 210 (44.1%) patients and 245 patients (55.9%) showed rectangular shape. Regarding WKG, 114 (25%) patients had width < 4 mm, 319 (70%) had width 4.1-8 mm and 23 (5%) patients had width >  8 mm. There was no significant difference between males and females for GT measurements. Regarding WKG, results showed that the prevalence of WKG 4.1-8 mm was more among females while males had more prevalence of ≤4 mm with significance difference. PH showed no significant differences between males and females. Regarding age, there was no significant differences between patients ≤25 years and >  25 years for all gingival parameters measurements. The relationship of smoking with different gingival parameters also showed no significant differences between smokers and non-smokers. Similarly, relationship of khat chewing with different gingival parameters showed no significant difference. Regarding inter-relationship between parameters, thin GT was associated with rectangular tooth form while square and quadrate forms are more associated with "1.5-2 mm" GT. WKG of ≤4 mm was associated with rectangular tooth form while WKG >  8 was more associated with square and quadrate forms with no significant difference. Results showed significant association between thin GT with 4.1-8 mm WKG. CONCLUSION: Yemeni population had more prevalence of "1.5-2 mm" GT, rectangular crown shape and WKG from 4.1-8 mm. Regarding interrelationship between gingival parameters, GT showed obvious relationship with WKG, CW/CL ratio and PH. WKG with CW/CL also showed significant relationship while no relationship was shown between other gingival phenotype parameters.

Introduction: The gingival biotype of a population is important epidemiological data for the best treatment planning, while preserving periodontal health. In Peru there is a shortage of similar data published. Objective: To determine the prevalence of gingival biotype in adult patients of the Dental Service of the Víctor Lazarte Echegaray and Hospital Regional Docente de Trujillo, April - June 2018. Ma-terials and Methods: This cross-sectional, descriptive and observational study included 200 patients from the Dentistry Service of the Víctor Lazarte Echegaray and Hospital Regional Docente de Trujillo. Using the accidental non-probabilistic selection method, the gingival biotype was evaluated through the transparency of periodontal probe technique. To determine the reliability of the method the Kappa statistic was used, finding an inter-evaluator reliability of 0.750 and intra-evaluator of 0.762. Descriptive statistics were used for the results, presenting absolute and percentage frequencies. Results: The prevalence of the thick gingival biotype was 57.5%. Males and females presented predominantly a thick biotype, 63.1% and 54.8% respectively. The groups of 18 to 29 and 30 to 39 years old presented 62.1% and 64.5% a thick biotype, respectively. On the other hand, the majority of people 40 to 50 years old presented a thin gingival biotype (52.8%). Conclusions: The thick gingival biotype was predominant in both sexes and in both hospitals. Also, gingival biotype varies with age, with the thick biotype more prevalent in younger age groups.


Introducción: El biotipo gingival de una población es importante como dato epidemiológico para la mejor planificación de tratamientos, preservando la salud periodontal.En Perú existe escasez de trabajos similares publicados. Objetivo: Determinar la prevalencia de biotipo gingival en pacientes adultos del Servicio de Odontología de los hospitales Víctor Lazarte Echegaray y Regional Docente de Trujillo. Abril ­ Junio 2018. Material y Metodos: El estudio transversal, descriptivo y observacional incluyó un total de 200 pacientes del Servicio de Odontología de los hospitales Víctor Lazarte Echegaray y Regional Docente de Trujillo. Utilizando el método de selección no probabilístico accidental, se evaluó el biotipo gingival a través del método de transparencia de la sonda. Para determinar la confiabilidad del método se empleó el estadístico Kappa, encontrando confiabilidades de 0.750 para interevaluador y 0.762 para intraevaluador. Se utilizó estadística descriptiva para los resultados, presen-tando frecuencias absolutas y porcentuales. Resultados: La prevalencia del biotipo gingival fue de 57.5% para el biotipo grueso. El sexo masculino y femenino presentaron predominantemente biotipo grueso, con 63.1% y 54.8% respectivamente. Los grupos de 18 a 29 y de 30 a 39 años tuvieron 62.1% y 64.5%, respectivamente de biotipo grueso. Por otra parte, de 40 a 50 años, destacó el biotipo gingival delgado con 52.8%. Conclusiones: El biotipo gingival predominante fue el grueso en ambos sexos y hospitales. Asimismo, el biotipo gingival varía con la edad, encontrando principalmente el biotipo grueso en los grupos etarios más jóvenes.

Subepithelial connective tissue graft (SCTG) is an essential therapeutic tool in periodontal plastic surgery and implantology. The aim of this preliminary study was to observe and make a histological and histomorphometric comparison of the composition of subepithelial connective tissue grafts (SCTGs) harvested from the palatal mucosa by two different harvesting techniques: mucoperiosteal (lamina propria and complete submucosa including periosteum) and mucosal (lamina propria and a portion of the submucosa). The main hypothesis proposes that SCTG harvested with the mucosal technique contains a greater proportion of connective tissue proper (CTP) and a lower proportion of adipose tissue (AT) than the mucoperiosteal technique. Twenty healthy patients who required SCTG for different purposes were selected and assigned to one of the two following groups: group A (n=10; mucoperiosteal harvesting technique) and group B (n=10, mucosal harvesting technique). The histological sample was obtained by removing a 2 mm thick slice from the most distal portion of the graft. The proportions of adipose tissue (AT), connective tissue proper (CTP) and vascular tissue (VT) were evaluated. In group A, histomorphometric analysis showed that CTP accounted for 58.2% of the graft while AT accounted for 32.64%. In group B, the proportions of CTP and AT were 79.86% and 11.93%, respectively. The differences between groups were statistically significant for both tissues (p< .05). In contrast, no statistically significant difference was observed in the proportion of VT. Within the limitations of this study, the results show that the SCTGs harvested by the mucosal technique contain a greater proportion of CTP and a lower proportion of AT than those obtained by the mucoperiosteal technique, whereas the proportion of VT does not differ. Further long-term clinical and histological studies with more samples are needed to evaluate the clinical implications of SCTG composition.


El injerto de tejido conectivo subepitelial (ITCSE) es una herramienta indispensable en la cirugía plástica periodontal y la implantología. El objetivo del presente estudio preliminar fue observar y comparar histológica e histomorfometricamente la composición de los injertos de tejido conectivo subepitelial (ITCSE) obtenidos de la mucosa palatina mediante dos técnicas diferentes: mucoperióstica (lamina propia y submucosa incluyendo el periostio) y mucosa (lámina propia y parte de la submucosa). La principal hipótesis postula que el ITCSE obtenido mediante la técnica mucosa contiene mayor proporción de tejido conectivo propiamente dicho (TCP) y menor proporción de tejido adiposo (TA) que el obtenido mediante la técnica mucoperióstica. El presente estudio incluyó veinte pacientes sanos que requerían ITCSE por diferentes motivos, los cuales fueron distribuidos de forma equitativa en dos grupos: grupo A (n=10; técnica de obtención mucoperióstica) y grupo B (n=10; técnica de obtención mucosa). La muestra histológica se obtuvo removiendo una porción de 2 mm de ancho de la parte más distal del injerto. Se evaluó la proporción (%) de tejido adiposo (TA), tejido conectivo propiamente dicho (TCP) y tejido vascular (TV). En el grupo A, el análisis histomorfométrico mostró que el TCP constituía el 58.2% del tejido mientras que el tejido adiposo constituía el 32.64%. En el grupo B, la proporción de TCP y AT fue 79.86%y 11.93%, respectivamente. Las diferencias observadas entre los grupos fueron estadísticamente significativas para ambos tejidos (p< .05). En cambio, no se observaron diferencias estadísticamente significativas en la proporción de TV. Dentro de las limitaciones del presente estudio, los resultados mostraron que los ITCSE obtenidos mediante la técnica mucosa contienen mayor proporción de TCP y menor proporción de TA que los obtenidos con la técnica mucoperióstica, mientras que el TV permanece estable. Se requieren estudios longitudinales clínicos e histológicos a largo plazo con mayor cantidad de muestras para evaluar las implicancias clínicas de la composición del ITCSE.

OBJECTIVES: The aim of this work was to compare the duration of treatment between orthodontic treatment combined with piezocorticision (OT-PC) and conventional orthodontic treatment (COT), as well as to evaluate the safety, inflammatory process, periodontal health, and soft tissue healing in the OT-PC group. METHODS: Twelve patients were included in each group, and their treatment times were compared for preliminary bracket alignment (PBA) and for overall treatment. In the OT-PC group, the inflammatory process was evaluated by quantifying cytokines in the gingival crevicular fluid. A calibrated examiner measured the probing depth (PD), the distance between the gingival margin and the cementoenamel junction (GM-CEJ), and the clinical attachment level (CAL), before and after OT-PC. The presence of gingival scars was evaluated. Bone and root injuries were assessed with the use of cone-beam computed tomography. RESULTS: The treatment time was significantly reduced in the OT-PC group for PBA in both maxilla (45%; P = 0.001) and mandible (31%; P = 0.023), as well as for overall treatment (52%; P < 0.0001). Although not statistically significant, several inflammatory mediators demonstrated peaks at 3-5 and 16 weeks. There were not significant changes in PD and GM-CEJ after OT-PC treatment, unlike CAL (0.09 ± 0.12 mm; P = 0.024); 47.5% of piezocorticisions caused gingival scars. Only one patient showed no scars. Four cortical bones did not heal completely, and 2 roots had piezoelectric injuries. CONCLUSION: OT-PC was effective at reducing the orthodontic treatment time.

The aims of the study were: to describe the gingival color surrounding the upper incisors in three sites in the keratinized gingiva, analyzing the effect of possible factors which modulate (socio-demographic and behavioral) intersubject variability; to study whether the gingiva color is the same in all three locations and to describe intrasubject color differences in the keratinized gingiva band. Using the CIELAB color system, three reference areas (free gingival margin, keratinized gingival body, and birth or upper part of the keratinized gingiva) were studied in 259 individuals, as well as the related socio-demographic factors, oral habits and the chronic intake of medication. Shadepilot™ spectrophotometer was used. Descriptive and inferential statistical analysis was performed. There are statistically significant differences between males and females for coordinates L* and a* in the middle and free gingival margin. For the b* coordinate, there are differences between males and females in the three locations studied (p < 0.05). The minimum and maximum coordinates in which the CIELAB natural gingival space is delimited are L* minima 28.3, L* maximum 65.4, a* minimum 11.1, a* maximum 37.2, b* minimum 6.9, and b* maximum 25.2*. Age, smoking, and the chronic intake of medication had no significant effect on gum color. There are perceptible color differences within the keratinized gingiva band. These chromatic differences must be taken into account if the prosthetic characterization of gingival tissue is to be considered acceptable. There are significant differences between the color coordinates of the three sites studied in the keratinized gingiva of men and women.

ABSTRACT Introduction: Maxillary incisal display is one of the most important attributes of smile esthetics. Objective: The aim of this study was to determine the relationship between maxillary incisal display at rest (MIDR) and various soft tissue, hard tissue and dental components. Methods: A cross-sectional study was conducted on 150 subjects (75 males, 75 females) aged 18-30 years. The MIDR was recorded from the pretreatment orthodontic records. The following parameters were assessed on lateral cephalograms: ANB angle, mandibular plane angle, palatal plane angle, lower anterior and total anterior facial heights, upper incisor inclination, upper anterior dentoalveolar height, and upper lip length, thickness and protrusion. The relationship between MIDR and various skeletal, dental and soft tissue components was assessed using linear regression analyses. Results: The mean MIDR was significantly greater in females than males (p = 0.011). A significant positive correlation was found between MIDR and ANB angle, mandibular plane angle and lower anterior facial height. A significant negative correlation was found between MIDR and upper lip length and thickness. Linear regression analysis showed that upper lip length was the strongest predictor of MIDR, explaining 29.7% of variance in MIDR. A multiple linear regression model based on mandibular plane angle, lower anterior facial height, upper lip length and upper lip thickness explained about 63.4% of variance in MIDR. Conclusions: Incisal display at rest was generally greater in females than males. Multiple factors play a role in determining MIDR, nevertheless upper lip length was found to be the strongest predictor of variations in MIDR.


RESUMO Introdução: a exposição dos incisivos superiores é um dos fatores mais importantes na estética do sorriso. Objetivo: o objetivo desse estudo foi determinar a relação entre a exposição dos incisivos superiores em repouso (EISR) e diferentes componentes em tecidos moles, tecidos duros e dentários. Métodos: um estudo transversal foi conduzido com 150 pacientes (75 homens, 75 mulheres), com idades entre 18 e 30 anos. A EISR foi obtida nos registros pré-tratamento ortodôntico. Os seguintes parâmetros foram analisados nas radiografias laterais: ângulo ANB; ângulo do plano mandibular; ângulo do plano palatino; altura facial anterior inferior; altura facial anterior total; inclinação dos incisivos superiores; altura dentoalveolar anterior superior; comprimento, espessura e protrusão do lábio superior. A relação entre a EISR e os diferentes componentes esqueléticos, dentários e em tecidos moles foi analisada por meio de uma análise de regressão linear. Resultados: a EISR média foi significativamente maior nas mulheres do que nos homens (p= 0,011). Foi encontrada correlação positiva significativa entre a EISR e: o ângulo ANB, ângulo do plano mandibular e a altura facial anterior inferior. Uma correlação negativa significativa foi encontrada entre a EISR e o comprimento e a espessura do lábio superior. A análise de regressão linear mostrou que o comprimento do lábio superior foi o maior preditor da EISR, explicando 29,7% das variações na exposição dos incisivos superiores em repouso. Um modelo de regressão linear múltipla baseado no ângulo do plano mandibular, na altura facial anterior inferior, comprimento do lábio superior e espessura do lábio superior explicou cerca de 63,4% das variações na EISR. Conclusões: a exposição dos incisivos em repouso foi, de um modo geral, maior nas mulheres do que nos homens. Múltiplos fatores exercem influência na quantidade da exposição dos incisivos superiores em repouso; porém, o comprimento do lábio superior foi identificado como o maior preditor das variações na EISR.

BACKGROUND: Successful periodontal and implant surgery as well as orthodontic treatment often depends on gingival and mucosal thickness. So far there has been no generally accepted protocol of measuring the thickness of gingiva by non-invasive methods. OBJECTIVES: The aim of the study was to evaluate the repeatability and reproducibility (%R&R) of the 20 MHz A-Scan ultrasonic device in measuring gingival thickness (GT) in the mucogingival complex. MATERIAL AND METHODS: A 2-stage study utilizing non-invasive ultrasonic methods was conducted. In the 1st stage, 3 operators got calibrated by measuring previously established GT in porcine cadaver jaws. In the 2nd stage, 1 periodontally healthy subject was recruited in the study. Three operators performed the measurements of GT in maxillary left central and lateral incisors and canines, using the 20 MHz A-Scan ultrasonic device with a probe of 1.7 mm in diameter. The thickness was measured in 4 standardized points located in the free gingiva (FGT), supracrestal gingiva (SGT), crestal gingiva (CGT) and the mucosa (MGT). RESULTS: The analysis of variance (ANOVA) method was used to quantify %R&R. The repeatability and reproducibility of the measurements was 8.4%. Interobserver reproducibility varied from 0.8% to 13.4%. The average intraobserver coefficient of variation (CV) was 6.6% (1.9-13.6%). The median of the reproducibility of all measurements was 8.1 %. Nevertheless, the median of CV was variable to the observer, i.e. 5.4%, 6.5%, 6.4%. CONCLUSIONS: The obtained results in %R&R prove the good recognition of methodology as well as the usefulness of the device. Non-invasive ultrasonic biometer GT measurements are crucial in periodontology as well as in other fields of dentistry.

ABSTRACT Objective: To evaluate the smile attractiveness of different gingival zeniths by general dentists, orthodontists and laypersons and the esthetic perception in the symmetric and asymmetric changes in gingival zeniths. Methods: Posed photographs of five patients were taken and digitally manipulated in Keynote software, in the gingival zenith region, in increments of 0.5 to 1mm in maxillary central and lateral incisors, symmetrically and asymmetrically, in nine different ways for each patient. The photos were then uploaded to a website, where evaluators (general dentists, orthodontists and laypersons) could observe and vote according to their esthetic perception, scoring from 1 to 10, 1 being the least attractive and 10 the more attractive. Kruskal-Wallis and Mann-Whitney tests were used for comparison. Results: Asymmetric gingival zeniths were less attractive than symmetrical gingival zeniths; gingival zenith differences greater than 1mm were perceptible in the smile attractiveness, both by laypersons, general dentists and orthodontists. When comparing maxillary central incisors with maxillary lateral incisors, the aesthetic change performed in the central incisors are more perceptible than those performed in lateral incisors, both symmetrical and asymmetrical. In a general way, orthodontists and general dentists are more critical in the evaluation and perception of gingival zenith changes, with the laypersons perceiving this change only from 1mm of maxillary right central incisor asymmetrical change. Conclusions: Asymmetric gingival zeniths are less attractive than symmetrical ones. Gingival zenith differences greater than 1mm are perceptible in the smile attractiveness. Orthodontists and general dentists are more critical in evaluating smile esthetics.


RESUMO Objetivo: avaliar a atratividade e a percepção estética do sorriso com alterações simétricas e assimétricas dos zênites gengivais, por leigos, dentistas e ortodontistas. Métodos: foram tiradas fotografias posadas de cinco pacientes, de forma padronizada. Cada fotografia foi digitalmente manipulada no software Keynote, na região do zênite gengival, em incrementos de 0,5 a 1 mm, nos incisivos centrais e laterais superiores, de maneira simétrica e assimétrica, de nove formas diferentes para cada paciente. Posteriormente, essas fotografias foram dispostas em um website para que os avaliadores pudessem observá-las e, conforme sua percepção estética, dessem notas de 1 a 10, sendo 1 para pouco atrativa e 10 para muito atrativa. Foram utilizados os testes Kruskal-Wallis e Mann-Whitney para as comparações. Resultados: os zênites gengivais assimétricos foram considerados menos atrativos do que os simétricos. Diferenças entre os zênites gengivais maiores que 1 mm foram perceptíveis na atratividade do sorriso por todos os grupos de avaliadores. Quando comparados os incisivos centrais e laterais superiores, as alterações estéticas feitas nos incisivos centrais foram mais perceptíveis do que as feitas nos laterais, tanto as simétricas quanto as assimétricas. Os ortodontistas e os clínicos gerais foram mais críticos ao avaliar e perceber as alterações, com os leigos percebendo essa mudança somente a partir de 1 mm de alteração assimétrica no incisivo central superior direito. Conclusões: zênites gengivais assimétricos são menos atrativos do que os simétricos. Diferenças nos zênites maiores que 1 mm são perceptíveis na atratividade do sorriso. Ortodontistas e clínicos gerais são mais críticos ao avaliar sorrisos.

OBJECTIVE: This study aimed to explore the relationship among gingival thickness (GT), underlying alveolar bone thickness (BT), and sagittal root position in the maxillary anterior measured by cone-beam computed tomography (CBCT). The theoretical foundation was applied to aesthetic dentistry, implant treatment planning, and therapeutic effect assessment. METHODS: A total of 40 young volunteers with healthy periodontal status were involved in this research [16 males and 24 females aged 23-34 years with a mean age of (26.30±2.29) years]. Three records were measured by CBCT. GT was measured at the cemento-enamel junction level. Buccal BT was measured at three locations: 1, 3, and 5 mm below the alveolar crest, along the sagittal angle between the long axis of teeth, and along the long axis of the respective alveolar bone. RESULTS: The average GT and alveolar BT thicknesses were (1.08±0.34) mm and (0.79±0.29) mm, respectively. The average angle between teeth and alveolar bone was 18.01°±8.96°. BT was positively associated with GT (r=0.293, P=0.001). The BT of canine was positively associated with the angle between teeth and alveolar bone (r=0.457, P=0.003). CONCLUSIONS: BT was relatively thin. An angle was found between the long axis of teeth and that of the alveolar bone. BT was positively associated with GT. An accurate diagnosis of GT, underlying alveolar BT, and sagittal root position in the maxillary anterior is necessary before implant surgery to devise an appropriate implant treatment plan and achieve a predictable esthetic outcome.

AIM: To compare the soft tissue volume gain (VG) around single tooth implants with subepithelial connective tissue graft (SCTG) from either the lateral palate (LP) or from the tuberosity area (TA). METHODS: Thirty-two patients with 36 implants with buccal volume deficiencies were randomly assigned to receive SCTG from LP (control group/CG) or TA (test group/TG). Clinical parameters were recorded. VG was evaluated by stereolithography (STL) image superimposition of two intraoral scans (baseline/BL and 3 months after surgery/FU-3). Descriptive analysis was performed for both groups, and for comparisons, Mann-Whitney U test was used. RESULTS: In terms of VG values, no statistically significant differences were observed except for values at 6 and 7 mm apically to the healing abutment which favoured the TG. Mean values were 0.69 ± 0.23 mm for CG while TG obtained 0.79 ± 0.10 mm (p = .64). Regarding Keratinized tissue (KT) width statistical significant differences were found favouring TG, which obtained a gain of 0.83 ± 0.61 mm compared with 0.22 ± 0.48 mm for CG (p = .009). Pink esthetic scores resulted in mean values of 10.07 ± 2.19 for the CG, while TG obtained 9.15 ± 2.34. CONCLUSIONS: Both procedures were effective in increasing soft tissue volume with no statistically significant differences. A longer follow-up is needed to confirm or refute these results.

Preservation or regeneration of the papilla has always been a challenge around consecutive implants or with implants next to teeth, and many studies have evaluated the papilla's behaviour and patterns based on surgical technique and prosthetic design, though evidence about its behaviour around zirconia implants is scarce. The aim of this study was to evaluate papilla behaviour between implants and teeth (tooth-implant group) and between consecutive implants (implant-implant group). Ninety patients with 122 zirconia implants (Straumann® PURE Ceramic Implant) were examined at the one-year follow up. We measured the effect of the distance: first from the base of the contact point of the crowns to the contact with bone at the implant site (D1); secondly, to the contact with the bone at the neighbouring tooth or implant site (D2); and thirdly on the papillary deficit (D3). In both the tooth-implant group and the implant-implant group, D1 and D2 correlated significantly with the papillary deficit (D3), whereas D2 was the major determinant factor (Spearman's rho=0.60). In both groups, when D1 and D2 were <6mm, the papilla was present every time. The papillary deficit was significantly greater in the tooth-implant group than in the implant-implant group (p=0.048). We conclude that the ideal distance from the base of the contact point to the bone contact at the implant and to the bone contact at the adjacent tooth in both groups is <6mm. The height of the bone on the teeth adjacent to implants has a significant impact on that of the papilla.
